If you are unsure what to do, you should e-mail the Instructional Support Coordinator (ISC) as soon as possible. Waiting until after your situation has passed may mean we cannot provide a suitable accommodation.
Do not contact course instructors or ISAs for accommodation requests. All such requests must go through the course ISC.
If for any reason you are unable to submit an assignment on Crowdmark, e-mail the assignment to the course ISC and they can upload your assignment for you.
If you declare a 48-hour absence on Quest, afterwards e-mail the course ISC to receive a 48-hour extension on an affected assignment.
For the midterm, its weight is shifted onto the final exam portion of the grade.
There is no accommodation for missed lecture clicker questions since the clicker policy has a built-in accommodation for possible absences throughout the term.
For student's suffering from any symptoms that could possibly (even remotely) be associated with COVID-19, you can fill out a self-declaration form on Quest. No Verification of Illness form or doctor's note is required in this situation. You MUST fill out the form as soon as you have symptoms as the self-declaration form will not allow you to backdate. (Do NOT wait until after you are better to fill out the form as you will be unable to date it for when you were ill.) You can leave the end date blank and fill it in on Quest once you recover. Students can find the Illness self-declaration form in the Personal Information section of Quest. After completing the self-declaration form, e-mail the Instructional Support Coordinator and explain your situation. No accommodation will be given until the self-declaration is registered on Quest.
For other illnesses that are not COVID-19 related, a Verification of Illness Form (VIF) is required.
If you are in the MATH faculty, see the following webpage for information:
https://uwaterloo.ca/math/accommodations/submission.
Your VIF must be submitting via the VIF webpage.
E-mail the course ISC after you have submitted the VIF.
Only after the VIF has been registered into the VIF system will an accommodation be given.
Note: You will be required to provide formal documentation when filling out the online VIF.
For doctor's notes, you can have the following VIF filled out:
https://uwaterloo.ca/campus-wellness/sites/ca.campus-wellness/files/uploads/files/verification_of_illness_form.v2_use.pdf.
If you are in another faculty, e-mail your VIF directly to the course ISC and also mention which faculty you are in
(otherwise we will assume you are in the MATH faculty and process your VIF incorrectly).
If you suffer from a power outage that prevents you from doing coursework, you should e-mail the course ISC including your power company, your location, and the time of the outage. Most power companies will post on their webpage or on social media about outages that we can confirm with. If possible, please send links to such posts.
For students that have to fly during the term, we do not make accommodations unless for emergencies. Students are expected not to be moving between locations during the term. Should you choose to take a flight, you are responsible for the extra time required to stay on top of the material and to make arrangements to complete all course assessments. Issues that are part of a flight such as getting visas or going through quarantine (where internet access may be unavailable) will generally not be considered if you choose to fly.
You should contact the course ISC well before your flight if you must fly somewhere during the term.
For all other scenarios where a student cannot complete coursework, e-mail the Instructional Support Coordinator
and explain your situation.
Note: Formal documentation of some sort is needed.
Please include such documentation if possible when contacting the course ISC (or arrange to submit such documentation later on).
Requests without some kind of formal documentation will not be granted.
